David Slade didn't make up the term, it's been around for years. It was traditionally a single frame inserted into the film often for advertising. The porn shot that Tyler Durden puts in at the cinema in Fight Club is a subliminal shot.

If films credited all the extras then the end credits would go on forever. The credits for a film like Gandhi (which has 300,000 extras) would be longer than the film itself.

It usually works out at about 3 minutes of footage a day. Of course, they film more but most of that gets cut. Also, the typical cost of a big Hollywood blockbuster is about $10,000 a second.
